Signs and Symptoms

Piles can manifest with various symptoms, depending on their location and severity:

  • Pain or Discomfort: Especially while sitting or during bowel movements.
  • Bleeding: Bright red blood may appear on toilet paper or in the toilet bowl.
  • Itching or Irritation: In the anal region.
  • Swelling: Around the anus.
  • Lumps: A sensitive or painful lump may develop around the anus.
  • Leakage: Occasional leakage of feces.

Types of Piles

Piles are classified into two main types based on their location:

Internal Hemorrhoids

Located inside the rectum, they are usually painless but may cause bleeding.

External Hemorrhoids

Located under the skin around the anus, they can be painful and cause swelling and irritation.

Grades of Piles

Internal hemorrhoids are further classified into four grades based on their severity:

Grade I

Small swellings inside the lining of the anus, not visible externally.

Grade II

Larger than Grade I, these hemorrhoids may prolapse during bowel movements but return on their own.

Grade III

Prolapsed hemorrhoids that can be pushed back manually.

Grade IV

Permanently prolapsed hemorrhoids that cannot be pushed back and may require surgical intervention.

Treatment Options

There are several effective treatment options available for piles, ranging from lifestyle changes to advanced surgical procedures:

Lifestyle Changes

Increase fiber intake through diet or supplements. Stay hydrated by drinking plenty of water. Avoid straining during bowel movements. Exercise regularly to promote healthy bowel function.

Medications

Over-the-counter creams, ointments, and suppositories can provide relief from pain and itching. Prescription medications may be recommended for more severe cases.

Minimally Invasive Procedures

Rubber Band Ligation: Bands are placed around the base of hemorrhoids to cut off blood supply, causing them to shrink. Sclerotherapy: A solution is injected into hemorrhoids to shrink them. Infrared Coagulation: Infrared light is used to clot the blood supply to hemorrhoids.

Surgical Options

Minimally Invasive Procedure for Hemorrhoids (MIPH): Uses a stapling device to remove excess tissue and reposition hemorrhoids. Doppler-Guided Hemorrhoidal Artery Ligation and Recto Anal Repair (DGHAL-RAR): Uses Doppler ultrasound to ligate arteries supplying blood to hemorrhoids and repair prolapsed tissue. Suture Anopexy: Uses sutures to lift and secure prolapsed hemorrhoidal tissue.
